USF students must complete and submit the Transfer Course Verification Form to the Registrar’s office before taking courses at other colleges and universities. A student who earns eligible credit for courses at other institutions without first completing the Transfer Verification form may not have the transfer credit posted to the USF transcript. The signature of the student’s current advisor on the Transfer Verification form is required for the form to be processed. A course number and title needs to be included for each desired course. Except for IPFW or Ivy Tech courses, a copy of the course description also needs to be submitted with the application. Once approved, the student is responsible for the admission process to the other school and for having an official transcript sent in a sealed envelope to the USF Registrar’s Office when the course is completed.
Transferred courses will be recorded on your USF transcript as long as you earn a grade of “C” or better. Accepted credit hours are not included in the computation of your USF GPA.
If you transfer to USF, you may have course work earned at accredited institutions evaluated and posted to your USF transcript.

Allied Health, Nursing and Science majors have a five-year limit when transferring in science and pharmacology courses. Computer Information Systems courses taken more than five years prior to enrollment at USF are not transferable.
